Injury to Performance
Cleared to train, refused by trainers: how to help the clients others turn away

The short answer
A client cleared by a doctor but turned away by other trainers is not a client you take on courage. You take them on process. That means reading the diagnosis for what it actually limits, interviewing the person about how they want to live, then building backwards from that life to today's session. It also means knowing where your scope ends and being able to talk to their medical team like a professional. The FASTER Injury Recovery to Performance course is £1,800, or £300 deposit and 12 monthly payments of £150, plan total £2,100.
Key Takeaways
- Start from the life they want, not from level one of a generic progression.
- "It worked for me" is not evidence. It is the logical fallacy that damages clients.
- The goal is a medical team impressed enough to send you the next one.
The referral nobody wants
Somebody has been passed to you. Their doctor is happy for them to train. Three trainers have already found a reason not to.
You want to help. It also looks like a mess, and mess is where people get hurt.
Here is the thing worth naming. The reason they were turned away is rarely cruelty. It is that nobody taught those trainers a process, so all they had left was fear, and fear says no.
The trap of your own recovery
If you have been through something similar, careful.
Your recovery is a sample size of one, with your history, your job, your sleep, your tolerance. It felt like knowledge because it hurt.
"I was fine, so it must work" is not science. It is the fallacy that quietly ruins clients, and it is most tempting when you genuinely mean well.
Can you take this client?
Two questions, in order.
What does the diagnosis actually limit? Not what the word sounds like. What it means for load, range, speed and risk, for this person.
What do they want their life to look like? Not their goal on a form. What they want to do on a Saturday.
Then you build backwards from that Saturday to what you can do on Tuesday.
That is the opposite of the traditional model, which starts at level one and inches upward regardless of who is standing there.
You know the version. The bloke who deadlifts his 20kg bag into the locker, walks out, and is handed a 1kg dumbbell for his rotator cuff.
He probably also does that breathing. The one people only do in gyms when they pick things up. I tried it in my 24-hour Tesco lifting a frozen chicken and the staff just laughed at me.
The system that protects both of you
Four skills, and none of them optional.
Research. You will meet conditions you have not met before. You need to find out what is known, not what is popular.
Interviewing. Most of what limits this client will not be in the diagnosis. It is in what they are frightened of.
Precise movement analysis. Watching properly, in detail, live.
Building nuanced exercises. Not picking from a list. Building for the limit in front of you.
That last part matters more than it sounds. Analysing a client's movement against their own movement, on a good day and a tired day, is where the useful information lives.
Talking to the medical team
This is the part that turns one referral into ten.
If you can explain what you did, why you chose it, and what you are watching for, in language a physio recognises, you stop being a risk they tolerate. You become the person they send people to.
It also protects your client, and it protects you. Written down, reasoned, defensible.
Where your scope ends
You do not diagnose. You do not treat. You do not write a rehabilitation plan.
If a client reports continuing pain, training stops until somebody qualified has looked at them.
Knowing exactly where that line sits is not a limitation on your work. It is the thing that lets you work confidently right up to it.
Frequently asked questions
Can a personal trainer work with an injured client?
Yes, if the client has been cleared to exercise and you stay inside your scope. You cannot diagnose, treat, or write a rehabilitation plan. You can modify training around a known limit and progress it, with a process for deciding what is reasonable.
What do I do if a client is in pain during a session?
Stop the exercise, and if pain continues, stop training until a medical professional has cleared them again. Refer to their GP or physiotherapist. Record what happened, what you saw, and what you did.
How is this different from a GP referral course?
GP referral teaches you to receive a client through a scheme. Injury Recovery to Performance teaches you to research a case, interview for the life they want, analyse movement in detail, and build for it, then progress them back to performance.
What does the Injury Recovery to Performance course cost?
£1,800 paid in full, or £300 deposit and 12 monthly payments of £150, plan total £2,100. It counts as active learning, so your PT Manager access runs while you study and continues when you pass.
